A six-item set built on one evolving patient chart. Each item maps to one step of clinical judgment, and the chart adds new tabs (Nurses' Notes, Vital Signs, Labs, Orders, History) as the case unfolds.

- Strategy
- Read the newest chart tab first, then re-read the older tabs for trend. Answer the question being asked in that step only — do not jump ahead to interventions during a 'recognize cues' item.
- Example
- A 68-year-old post-op day 2 develops RR 26, HR 118, temp 38.4°C. Step 1 asks which findings require immediate follow-up; step 5 asks which two orders to implement first.
